How to Obtain a Passport

To obtain a passport, you must present the following to:

  1. COMPLETED APPLICATION FOR PASSPORT, DSP-11, including all the required information. The IRS now makes it mandatory for the Department of State to require Social Security information. If you proceed with the application without the information required by the IRS, the Department of State will still process the application and issue a passport. The IRS may contact you subsequently asking for the missing information. This also applies to minors.

    Applicants who do not have a Social Security Number should enter all zeros in the place of the Social Security Number. There is no penalty for passport applicants who have not been issued their Social Security Number. However, any applicant who fails to provide the required information may be subject to a $500 penalty enforced by the IRS.

    DO NOT SIGN APPLICATION until instructed to do so.
  2. Certified Copy of Birth Certificate bearing:
    • Raised Seal
    • File Date
    • Name correctly spelled
    • Date of Birth
    • Place of Birth
    • Signature of Registrar
    OR:

    Certificate of Naturalization or Most Recent Passport
  3. Two Identical Color Photos:
    2" x 2" on light-weight paper with white or off-white background, less than six (6) months old. Local photographers will take photos with correct size, paper and lighting - no uniforms or hats.

    Applicants under sixteen (16) years require both parents' signatures in our office.

Fees
For sixteen (16) years or older
$75.00; payable to U.S. Department of State, check or money order only
$25.00; payable to Washington County Clerk
Total: $100.00 (two (2) separate payments)

For fifteen (15) years or younger
$60.00; payable to U.S. Department of State, check or money order only
$25.00; payable to Washington County Clerk
Total: $85.00 (two (2) separate payments)

Passport renewals
You can send Form DSP-82 yourself. You must submit your most recent passport, and it must have been issued less than fifteen (15) years ago. You must have been sixteen (16) or older when the old passport was issued, and you must be using the same name as on your recent passport OR you have had your name change by marriage or court order and can submit proper documentation to reflect the name change. By sending this form yourself, you will not be required to pay the $25.00 processing fee to Washington County. The fee for this is $75.00

Approximately six (6) weeks routine - two (2) weeks expedited

Important Information

  • An adult passport (16 years of age and older) is valid for ten (10) years.
  • A passport for a person 0 - 15 years of age is valid for five (5) years.
  • Allow six (6) weeks for issuance of a passport.
  • If traveling before six (6) weeks, an expedited passport is needed (for an additional fee of $60.00 per passport)

Effective November 1, 2004: Whether DS-3053 form is used, or the same information is provided on other paper, effective November 1, 2004, the Statement of Consent by non-applying parents for children under the age of sixteen (16) must be notarized. The purpose is to prevent forgery and ensure that the person signing the Statement has been properly identified. This will further reduce the possibility of a U.S. passport being used in any effort to interfere with the custodial rights of non-applying parents.
